Position Purpose
This position is responsible to provide a full range of supported employment services to clients using the Individual Placement and Support Model and to educate ACT team members about supported employment. In addition, having responsibility to carry out rehabilitation and support functions and assist in treatment, education, support, and consultation to families; and crisis intervention in consultation with the multi disciplinary team.
Major Duties & Responsibilities

1. Engages clients and establishes trusting, collaborative relationships directed toward the goal of competitive employment in community job settings with other workers without psychiatric disabilities.
2. Assists clients in obtaining information about their benefits (e.g., SSI, Medicaid, etc.) and how they will be affected by employment in order for clients to make good decisions about employment opportunities. Helps clients to access benefits counseling when making changes in work hours and pay.
3. Assesses clients vocational functioning on an ongoing basis utilizing background information and work experiences.
4. With the clients permission, provides education and support to family members about work and gathers input about skills, interests, strengths of the client and ideas for support.
5. ACT Vocational Specialists are responsible for working as a team, in concert with each client to develop an individualized plan of services and supports (Individual Treatment Plan) that will enable each client to live as independently in the community as possible. Services and supports might include activities such as assisting with skill development in the following areas: budgeting, housing, transportation, hygiene, appointments, relationship building, symptom management, medication administration and obtaining or maintaining benefits.
6. Discusses clients preference for disclosure of psychiatric status to employers, including possible costs and benefits and specific information to be disclosed. Provides assistance with the job search regardless of client’s preferences regarding disclosure (i.e., support without employer contact when client chooses not to disclose). Assist in the provision of individual and group treatment in the office and in community.
7. Collaborate with the team to ensure the protection of clients’ rights in order to help clients improve their knowledge of client rights, and grievance and complaint procedures.
8. Assist clients with accessing and maintaining services by helping with forms and developing collaborative relationships with service providers.
9. Develop all treatment driven documentation within required deadlines.
10. Collaborate with, support, and provide education to family members and others interested in the client’s treatment and welfare.
11. Participate in the civil commitment process as mandated.
12. Participate in team meetings to provide input into the development of tx plans.
13. Follow treatment plans developed by the client/team.
14. Assist with and/or teach clients independent living skills such as cooking, cleaning, personal hygiene, transportation, and budgeting. Assist clients with making and keeping appointments.
15. Participate in daily staff organizational meetings and treatment planning review meetings.
16. Provide coverage (illness, client crisis) for team members to maintain continuity of client services.
17. Participate in professional development activities and share knowledge with the team.
18. Participate in scheduled agency staff meetings and trainings.
19. Document all work pertaining to client services to maintain quality assurance.
20. Maintain thorough, accurate, and timely client case records, reports, ITPs, and outcome surveys in accordance with data privacy requirements.
21. Complete all other paperwork consistent with the standards of MHR, community, and other legal requirements.
Other Duties - these are secondary functions of this position

Job Requirements
Position Qualifications
A. Minimum Education and Experience
Baccalaureate degree in social work/psychology or human services related field with two years case management experience of which at least one year is experience in supported employment related work or combination of equivalent experience and training.
B. Knowledge, Skills and Abilities
Ability to model behavior consistent with the Agency mission, values and code of ethics.
Knowledge and experience with intercultural competency as demonstrated in the ability to work effectively with all people.
Ability to establish constructive working relationships with clients and colleagues from diverse backgrounds.
Knowledge of severe and persistent mental illness signs, symptoms and treatments.
Knowledge of chemical dependency issues and treatment
Knowledge of mental health resources in the community.
Skill in assessment of client mental health symptoms and service needs.
Skill in crisis prevention and intervention.
Skill in effective written and verbal communication.
Ability to develop and use evidenced based practice to meet individual client needs.
Ability to work independently in the field and to use team resources/supervision to enhance performance.
Ability to transport clients and perform outreach in the community.
Ability to be flexible in prioritizing in a changing system.
Ability to work on-call, evenings and weekends as scheduled.
C. Working Conditions
Ability to work in an environment which requires the use of a personal computer, telephone, and other technical apparatus 30% of the time; ability to perform repetitive motions of the fingers, hands, and wrists.
