SUMMARY
The St. Paul office of BrightStar is continuing to grow and we are looking for another RN to help with our wonderful clients! This position will be an assistant to our Director of Nursing with the flexibility of10 to15 hours per week, based on the candidates availability. The successful candidate will be a Registered Nurse in good standing with the State of MNthat will perform initial client assessments, periodic supervisory visits of our existing clients, medication set-upsand provide other case management for our clients.Experience educating and training staff, a plus. At least1 year of home care experience is required; familiarity with Class A rulesis a plus.
This is a part-time position starting with 10-15hours per week plus2 weekends per month on-call and periodic help with the on-call phones during the week; schedule isflexible based on the candidates availability.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provides care as defined within the scope of the states nurse practice act. Makes the initial evaluation visit and regularly reevaluates the clientsneeds. Initiates the plan of care and makes necessary revisions. On-call duties 2 weekends a month and periodically as needed to back-up director of nursing. Prepares clinical and progress notes and summaries of care. Informs the physician and other personnel of changes in the patients condition and needs. Counsels the client and family in meeting nursing and related needs. Supervises, educates and coaches other nursing personnel, as appropriate.. Follows agency protocols to assureproper maintenance of client clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities. Identifies and participates in performance improvement activities, as requested by Director of Nursing. Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel. Promotes home health referrals in the health care community. SUPERVISORY RESPONSIBILITIES
Supervision of field staff performance of medical-related skills.
Job Requirements
EDUCATION and/or EXPERIENCE
Registered nurse with current unencumbered licensure to practice professional nursing in Minnesota. Preferred: Bachelors degree in Nursing. Required: 2 year nursing degree from accredited program. Minimumtwo (2) years of clinical experience with at least one yearworking in home health or related health care organization. Demonstrated ability to supervise and direct field staff. Possess exceptional nursing assessment skills Comply with accepted professional standards of practice. Excellent interpersonal skills. Analytic/Critical thinker. Excellent customer service skills and ability to deal tactfully with clients, staff and community. Treat clients, staff and the public with courtesy, respect and presents a positive public image. Case management skills are a plus Ability to travel on a daily basis including weekends, nights and holidays Understands regulatory compliance - MN Class Arequirements
- Federal regulations including but not limited to OSHA, HIPAA
- Joint Commission Accreditation, a plus
